    aus einem mailverkehr im mai 2011

    das kh scheibbs (90% aller zuweisungen) versteht es bis zum heutigen tage nicht (wir schreiben das jahr 2011), die (automatische) zuordenbarkeit von arztbriefen beim befundimport in meine ordinationssoftware anhand des patientennamens dadurch herzustellen, dass ein patient weißinger (mit scharfem „S“) als weissinger (mit doppel-S) aufgelöst wird, sondern schickt den befund als weiszinger aus (mit „SZ“). meine edv kann natürlich den arztbrief jetzt nicht automatisch zuordnen (auch die ecard verlangt ja einen korrekten namen, der mit dem in der EDV übereinstimmen muss). genauso bei jedem kröll, der als kroell nicht zuordenbar hereinkommt, genauso mit baerentaler statt bärentaler. das geht seit nunmehr 15 jahren(!) so dahin. ich glaube, ich habe es 250 mal urgiert , es blieb ohne ergebnis. die ecard software beruft sich auf standards, die ein kh nicht einhält (einzuhalten braucht)….

    in einem telefonat dieser tage wurde das nichtfunktionieren damit begründet, dass der aufwand der namensänderungen so hoch ist, weil das kh scheibbs, als vorreiter in der edv-steinzeit (o-ton) damals namensanpassungen gemacht hat, die bei jetziger änderung die patienten mit den damals zugewisenen schreibformen nicht mehr auffindbar machen würden…?

    mein kommentar dazu: wie finden denn die  im krankenhaus scheibbs eine frau mayer, die geheiratet hat und jetzt huber heisst?

Es sei denn, der Grundgedanke, dass der Mensch im Vordergrund steht und nicht die Erfüllung irgendwelcher bürokratischer Auflagen, tritt wieder in den Vordegrund…..

    Begründung:     die Ablehnung bzw. die (nichtvorhandene) Begründung empfinde ich persönlich als Skandal und behalte mir Schritte vor. Pat. war zusätzlich bei Orthopäden (in dieser Angelegenheit und bekam auch von dort nach synoptischer Betrachtung des Krankheitsbildes die Empfehlung zu „Protelos“)

    #2. ABLEHNUNG     -> Bewilligung von Protelos nicht möglich, da regelkonformer Einsatz.  .Seit wann Therapie mit Biphosphonaten ?? WK Fraktur vor 3 Jahren  ?  Es stehen mehrere Präp. bei WK Fraktur mit RE 1 Regel im EKO  zur Verfügung . Mit freundlichen Grüßen

    Begründung:     Bitte um Vorschlag, WAS ich der Pat. geben soll.Sie hatte vor >5 Jahren eine Magenblutung, allerdings keine Refluxkrankheit zuvor. Seit 2005 hatte Sie Rocaltrol und Alendronsäure, natürlich inkl oralem Kalzium, etc). trotzdem zunehmende Osteoporose-Auslenkung des T-Score(2003 nach QTC-Messung -5.0). Nach dieser Th-Dauer und erwiesener Unwirksamkeit sind (alle) Biphosphonate lt.EBM-Guidelines nicht mehr sinnvoll, ein Strontiumranelat (Protelos) m.E. das Vernünftigste. Prolia (Denosumab, kostet 250 Euro), Forsteo (Teriparatid, ein PTH-Analogon) kostet 400 Euro, Calcitonin ist erwiesenermaßen deutlich schwächer wirksam, als Biphosphonate, also aus naheliegenden Gründen genauso zu hinterfragen. Also bitte, WAS schlagen Sie vor? Nennen Sie doch Namen! Ich dokumentiere unseren Schriftverkehr ausführlich und werde das Ganze zur Diskussion stellen. Mir gehts nicht um Bewilligung ja/nein, mir gehts ums Aufzeigen, wie problematisch ABS ist..
